    Open your template.. . Make sure that the Developer tab is available to you. If it isn't, click on the Big Office Button in the upper left of the Word window then select Word Options. Leave all the text that belongs on all forms so that you have a generic document to work on.. . Put the cursor in the document where for example you are entering the patient name. A field will be inserted into your document. Now, click on Properties. Enter a default text if you think it necessary. Then set a maximum length for the field. Your experience should give you an idea of how long the field should be. Now under the Bookmark, give the field a name. For this example call it name. (if you have first and last names and perhaps an itital or middle name you should have a field for each of those). Under Text Format choose First Capital.. . Select Fill-in enabled and select Calculate on exit. Click OK.. . Put the cursor where you want the filled in text to repeat and on the Insert tab, Quick Parts, click on Field and scroll down to Ref and click on it. A box will open and the names that you assigned to the name field(s) will be presented. Select what you want to appear where the cursor is in your document template. In the Format field select First Capital and click OK.. . Repeat this process for each item that is repetitive in nature in the document. When finished, click on the Developer tab, click on Design Mode to de-activate it. Click on Protect Document. In item #2 Select Allow only this type of editing and select Filling in forms. Click on Yes, Start Enforcing Protection. Give a Password if you want. (you don't need one if you don't want one).

    I keep getting letters about this credit card which I never had and when I tell them they have got their wires crossed or something they still say they are going to pursue it. I have honestly never had this credit card, I COULD pay it if I wanted it's not that much they are claiming I owe them but why should I if I never had the card in the first place?? I have been getting these letters every now and again for a long while now it keeps getting moved to different debt collecting agencies. So the question is, how can I stop them pursuing this non debt? And yes I am completely sure I never had a credit card with them.. It's not a scam it's real debt collectors that exist I had heard of them before plus scams are usually obvious.. "don't pay simples" I'm not I want the letters to stop.. Identity theft? But wouldn't they max out the card if it was that? This is only less than ?300..
