My ex and I have a 6 year old daughter whom he sees once every few months. When I went on a trip to Europe a few years ago, I obtained a document from Legal Aid for us to sign stating we both agree my parents should get custody if something happens to me. The document was lost and never signed/notarized.. . However, I am about to have major surgery in December and my ex still agrees our daughter would be happier and more comfortable with my parents while he keeps his visitation.. . I don't expect him to change his mind, but I would still like to have a document stating that at this time, we both agreed she should be with my parents and that my wishes are known. I have a Advance Health Care Directive but I am more concerned about my daughter's guardianship. (as of now, I have full legal and physical custody I believe). . He is not unfit, nor am I attempting to make him look so. Our daughter just knows her grandparents much better and would be happier in their care.. . I need to know if a simple Power of Attorney is the best we can do, or if there is another option.. . Thank you for any help..

Medical Assistant colleges near Chicago IL?

I had a name my mother had but last minute she changed it and all my life I felt like the other name suit me best because I was named after a girl in a book that really has bugged me for the longest time. In California do I have to sign papers because I have heard I can usually just start using it but how would that work for official documents and college? Also I am worried about being harassed about it from friends and coworkers since the name is unusual especially for America (I was born in a different country)..

My uncle is helping my grandma in New York who most likely only has days to live. He has had POA for years, just in case. My grandmother's bank in CA will not honor the POA and is saying that if my grandma cannot sign *their* POA form, they require a local government official to attest to her inability to sign! That was the whole point of already having POA in place! Have you ever heard of something like this? What can be done?. They had POA established in NY for years.
