Where do you get power of attorney papers?

I have worked at Walmart for 4 months now. I was offered a postion at a Doctor's office and I want to take it. I will miss my coworkers, but I am pregnant and do not want to be standing behind a register all night. I work as a cashier and I am looking for sample resignation letters. I am giving my proper 2 week notice but don't want to write an ultra-professional letter for a cashier position. I don't want to close any doors. Thanks!.
