a lot of times letters of recommendation are literally just letters written by someone you know recommending you, meaning, they don't fill out a form. They just write a letter. If you are requesting one from someone you don't know very well (i hardly knew any of my professors in college), make sure you either provide them with a resume and list of accomplishments that they could include when writing it or, if that seems to pushy for your situation, schedule a meeting with them to sit down and talk about what your goals are. This provides a good opportunity to clarify things to make sure they aren't confused about what you have done, your grades, etc... Most college professors have done plenty of these with and without forms so they should know what to do. Just make sure you are clear to them about what its for and make sure you answer any questions they have. Good luck!
