I've left the link for you in the source list from which you may download it for free.. . And as for a model, if you mean template, it depends what kind of letter you need and then searching for that kind of letter should bring up some results on the web.

The college I really want to transfer to requires a Letter of Recommendation form to be filled out by a teacher or advisor. It just has questions that rate my performance and can easily be marked in a couple of minutes. The problem is that I have no idea who to give it to.. . I am supposed to be going into my third year of college, but I took a semester off last Fall and didn't do very well this Spring at my state university. I made excellent grades my first year but my professors wouldn't even remember me it's been so long, and I did well in high school but it's already been 2 years since I graduated. I have no idea who to contact about this. Last semester I dropped 3 courses and made B's in the remaining two, but one was online and the other was a 300 student lecture where I never even spoke to the professor.. . I don't know what I can do. I have a recommendation form that one of my high school teachers filled out when I was first applying to college but my other university has it. Do you think I could request a copy of it and send it to the new one? Or is it too old? What can I do? Can I fake one and get away with it?. . Any advice?.
