Have a care plan you can make your own but must be signed off by a solicitor. I have one for critical care , unconscious care , loss of mental capacity, general care. All state day by day request and wishes from what I'm to be bathed in soap wise to my preferred diet and how I'm always cold so need extra layers and what medication i am happy to take and what I'm not happy to take.
