When I got married, I took my husband's name. We ended up getting divorced (it ended good, we just weren't right for each other and are still friends), and I didn't change my name back. I had been in a business where my clients at my company only new me by my married name. I didn't want to change it back and have to explain it. Plus with all the systems and email and other programs that I would have had to change back, plus my license and my SS card...it just would have been a pain.. . If I get married again, I'll take my new husband's name. If my guy and I get married, I'd be proud to bear his name. And it has never bothered me when someone refers to a woman as Mrs. John Smith. I know there are people out there that don't believe in this, as the term Mrs. actually stemmed from "mister's", showing ownership of the wife in the technical sense. But it doesn't bother me. I guess I'm a little old fashioned when it comes to this. But you're marrying the person you love - why wouldn't you take their name?

I think for purposes of getting you enrolled in school and seeking medical treatment then you just need to have your parent fill out a power of attorney for the neighbors. You can get the forms off the internet or the local office supply store, and you need to have it notarized which cost from 5 -20 bucks depending on where you live.
