Careful. I say that because Dallas has a huge peasant underworld. Lot's of homeless, drugs, and crime. Lot's of criminal preying on innocent law abiding citizens since they can get away with it. Most of the power and money is centralized in the hands of a select few and their cronies. Some locals say that area governments depend on such a large poor population as a demand for it's social services. The federal, state, and local police are also very corrupt. Lot's of unreported human rights abuses by authorities towards citizens. Among other examples, A few days ago the Dallas district attorney was help contempt of court for not answering questions regarding an attempted shake down of a local citizen by government staff.

Charitable care hasn't gone away. As an emergency physician, I can have days in which I know at least a quarter of my patients won't be able to pay their bills. The good old days, by the way, weren't such good old days for those who were uninsured. They got care, but didn't have anything even close to the outcomes expected nowadays. I agree that over-regulation has squashed charity to too large an extent, but there's no way to turn back the clock. Medicine is simply too technologically based, and too expensive, now, and we expect miracles as a matter of routine.. One thing not being discussed, though, that could have a huge positive impact would be to remove medical malpractice from the tort system. It wouldn't be hard to do away altogether with the idea of the malpractice suit, with compensation for injuries coming from a general pool and requiring only that there be an injury to qualify for compensation. That removes the adversarial role and allows a little more room for physicians to work more cost-effectively. It works in New Zealand.
