Hi,. does anybody know where I can find a power of attorney form download? I prefer pdf format and it should be for free, if possible..

If you don't hear back, a polite follow-up in a week is a good idea. You can inquire to make sure he got the letter, and verbally express your interest again at that time. If he says he hasn't thought about it yet, it is okay to ask when he plans to make a decision. (That way you can keep in touch but not be a pest -- maybe he says he is traveling or busy finishing another project and will not think about this for three weeks. Then you'd know to wait for that length time and informally follow up again, but not before.). . Good luck!

Typically when writing a formal letter, you are writing with a purpose. You grammar and vocabulary should reflect this purpose

No, it means your maiden name. For an example it asks all previous names known under, and your maiden name is Smith, put down Smith. Also it goes for nick names as well, like some jobs called me Jackie and others go by my real name.

Can I change my name back to my birth name without doing it through deed poll ?
