birth cert - original name, marriage cert - married name, deed poll - changed name, for places like banks etc they will require proof of this. If your not seperated you 'll have to fib tho and say you are or it's long they just won't understand why you want to do it, no point standing there explaining lol! You don't have to be divorced. Many people don't ever get divorced. Your birth cert and marriage cert is proof enough for this ok.

You do not need a template. All you should say in a resignation letters is that you are resigning as of a particular date. Do not say ANYTHING else. It could come back to haunt you. No reasons, no excuses no explanations of what you are going to do next. NOTHNG.. . Seriously, that is what HR experts all say.. . Look at when Richard Nixon resigned from the Presidency.. All he said in his letter to the Secretary of State was "I hereby resign the office of President of the United States as of noon, August 21, 1974.". .
