birth cert - original name, marriage cert - married name, deed poll - changed name, for places like banks etc they will require proof of this. If your not seperated you 'll have to fib tho and say you are or it's long they just won't understand why you want to do it, no point standing there explaining lol! You don't have to be divorced.
