  • Mary C. Scott-Hattendorf is an associate in the firm's San Diego office. She started as a law clerk with the firm in 2009. I will be getting married soon and both my fiance and myself want my oldest daughter to have his last name. I m just not sure how I would go about doing that? Unfortunately her biological father hasn't been there since day 1 -- but my fiance has raised her since she was born as his own.... we want to make it official but I heard I need her biological father permission in doing so, is this true? right now she has my last name..... i want us all to have the same last name, anyone have any experience with this???. . Even better, go to a bookstore and look in the section on legal books, and you will probably find a book of legal forms which will include POA's. Either buy the book, or just copy (transcribe) the document, and go home and type it up on your computer. As long as it has the basic legal elements to it, it will work. You can so the same for a sample form that you find on the internet.
