Our school's career counselor asked me to write a thank you letter to the employer for accepting me for work experience. I was accepted into a design company and I'm asking for some guidance and maybe a template for writing this letter.

For an assignment, we have to create a cover letter for a fictional job. The person it's suppost to be addressed to is the head of the business faculty at my university.. . I've actually never written a cover letter before, but our prof was purposely unclear on how to create a cover letter, saying something about real life blah blah blah. So I've looked up templates online, and they all say to include the name, job title, company name, and address/ city/ province/ postal code.. . So... does that mean the address of the person whom the cover letter is addressed to, or the address of the company they're affiliated with?.

Be very leary of form letters. They tend to do more harm than good. You don't need anything fancy. Just simply say "I withdraw my authorization for (insurance company) to debit my account.". . In the future you may want to avoid allowing creditors to auto debit your bank account. I recommend checking into you bank or credit union's online billpay. Most offer it at no charge. You can set up automatic payments for recurring things like insurance or rent. Yet you still have control and can change or stop any payment up to a couple days before the due date.
