Simple as that, a legal document confirming an intention. Nothing more, nothing less.. . As a legal document a deed poll needs to be correctly worded/laid out. Before the Internet if you needed a legal document you would go to a Solicitor who would know the correct wording to use.. . These days you don't need to go to a Solicitor anymore, instead you can use a specialist online website (prices range from free upwards), or you can purchase a template, or if you find the wording you can type it up yourself!. . I hope this is helpful, remember in a nutshell - to change your name you just start using a different one, and to use that name on all of your records, you just need a correctly worded legal document called a deed poll.

I know you can find a "power of attorney" form online. You may also be able to find a "legal guardianship" form. I think to change an actual custody agreement, you have to go through the court. But signing over legal guardianship to your dad would let him register you for school, etc.

My fiancee has cancer. He has no will and no children. He has two fairly large debts. He wants me to have the ability to make medical decisions now and any other decisions regarding organ donation and cremation after his death. He has very few assets. Would it be easier for me to have power of attorney or for us to get married before he dies? If we get married, would that make me responsible for his debts? Is it necessary for him to have a will?.
