how to write a letter requesting emergency leave from work?

Estate issues? What to do if executor is exploiting estate and/or not properly overseeing?

I have to give a 60 day written notice prior to vacating my apartment in florida, and i cannot find any forms or anything. does anyone know what this notice is supposed to say, or where to find a template for one?.

I've read all of the answers on the site but I'm still confused. I just got married in Brooklyn, NY. I had originally changed my name on my marriage license to a hyphenated name so my official marriage license has a new surname on it. But I've since changed my mind and want to change my middle name to my maiden and last name to my husbands. Is this a court thing? or can I follow the go to your SS office path? Thanks!.

Microsoft word template problem?

I have my husband's last name but now I want to change to my family last name, I didn't divorce, I just want to change it. How do I do that?.
