In Illinois legal name changes are handled by the circuit court in your county. Check their website for fees and the exact process. The process varies for minors and adults.. . Before going to Social Security, DMV, etc. you have to file for and obtain a legal name change and have documents proving your prior and new names. Don't go to any place that isses ID until AFTER you've done the court process. Once you've gone through those two steps, you will need to change your accounts as well.

Its really challenging and you need to study consistently if you wanna succeed in STPM... For science stream, form 6 classes can be divided into two. Biology class for those who take Biology and Physics class for those who take Physics of course. So, naturally, people in our form have the same lessons except for the Bio/Physics lessons. If the hyphenated name is considered a defferent name then I would think you were in the clear. But I don't know what your decree says. Maybe you should have your lawyer interpret that before you go through all the hassle to change your name to something that might cause more negative consequences than good ones.
