No, changing you name at any age is not unlawful unless you are trying to avoid possible prosecution for crimes done in your rightful name. You and Mother should just visit a competent attorney and ask for a name change - that would affect both of you. Then you would have to change your name on SS Card (your mother would also change her name on her SS card) You would go to a new school and show them your new ID - State issued. And you Mother would have to show the paperwork (judgment from court) to DMV to change her DL.
My ex wife left 2 of our children and 1 of her children with me about 2months ago and has not came back for them. She also moved out of state illegally with our oldest daughter. I am trying to get modification of custody, so that I may have sole physical custody of all of the children. I have looked on my states website, asked the secretary at the court and looked all over the internet and can not find the correct forms to file for custody in the state of Indiana. I can't afford an attorney and have already presented my case to pro bono lawyers with no luck. Can anyone tell me what forms to fill out for modification of child custody in the state of Indiana?.
I just recently had my name changed and I know the next step is to change it on my name on my social security but I am a little clueless as to going about doing this. I don't know where to do this at or who to go to. I live in greenville NC and I've tried looking up this information only to end up confusing myself. I don't know who to go to or who to ask, so if you have any information please give me some some advice or at the very least some direction..
There isn't. Just write a formal cover letter of application.
