I did my elderly mother's taxes for a number of years. Her income included Social Security, her survivor's pension from her husband's work, and some interest and dividends. She had tax withheld from the pension payments, and every year, she got all of it back. When her house was sold, the home sale capital gains exclusion didn't quite cover the increase, and the withholding covered her tax liability.. . So, she may not owe anything, and there are no interest or penalties if tax is not owed. But if she does owe, you probably want to take care of it sooner rather than later, since the amount owed will continue to increase.. . And she doesn't have to be declared incompetent for you to help her with her taxes. You can prepare them, then you would review them with her and have her sign.. . On a non-tax-related note - before she becomes incompetent, I suggest that she execute a durable power of attorney (including healthcare) so you can take care of this kind of stuff for her when she is no longer able to. [Note, you can't do a DPOA after she becomes incompetent, and having guardianship declared afterward requires going to court. PITA.]
