I am 80 years old and just found out that the last name that I have been using is not the name on my birth certificate. I was raised by my grandmother and had never seem my birth certificate. All my important papers are in the name I have been using. My social security card my military records, bank accounts and drivers license everything. So how do I go about changing my birth certificate to the name that I have always assumed was mine. I was born in Florida and I now reside in New York state.
