I filed a Petition for Change of Name for my daughter to change her last name from that of her fathers' to mine. I received a notice which states that in order for the file to be summited to the judge for ruling they need to receive the following 3 documents.... -Failure to complete with Maryland Rule 15-901(d). -Petition and notice must be served on minor child's father. -Affidavit of Service must be filed. . When filing I was told that the father merely had to be informed of the name change but not necessarily consenting. If he did not agree he could appeal, but I only had to provide proof that he was informed. I would mail him the papers by certified mail and the green card that comes back would be my proof.. . I called the court to make sure that the green card would be enough but was toled that I would also need a letter giving his consent.. . Two different people from the same place telling me two different things.. Getting the letter of consent would be more difficult than sending the papers certified.

Big Law Firm. . 1) If your business is not BIG, then you are going to be low priority for the Big Law Firm.. . 2) They are going to bill you A LOT. So you need to figure out if you can afford them.. . 3) Having Big Law Firm doesn't mean you are going to win every case: facts are facts and they can't force the judge/jury to ignore the truth.. . Small Law Firm. . 1) Their lawyers might be as good as those from the Big Law Firm

You don't need a notary for your will. Just 2 witnesses. Put the complete names and addresses and phones of the witnesses somewhere so they can be contacted if someone says your will is not valid.

I'm sure you mean power of attorney and no you don't. My state has a form you can download or you can buy the form from a copy store or stationary shop. If you want to do something that doesn't guite fit within the statutory form, it would be wise to have a revision/modification done by a lawyer.
