The truth is, most schools have their applications on line in PDF format. Go to your guidance counselor (or library) and look at the catalogs from the schools you are interested in. Or, go online to the website of each school and download their application. No one is going to care what your request for an application looks like. As a matter of fact, most schools would prefer that you send them a postcard with your information on it. It's what ON the application, your transcripts, your references, and your personal statement that they will look at.
