Format Of Formal Letter !!!!!?

Btw, Im sure my parents will say yes. Im just scared they wont agree..

Just getting in the door is the hard part so I've designed a single page informational advertisement to send to HR Managers in hopes of getting some interest.. . Any thoughts or help would be greatly appreciated.. . Chuck.

In the US, generally you will fill out the birth certificate form yourself, so you can put your married name on the certificate. Most states (I'm in California) don't require a name change through court if you're changing it because of marriage.. . And there's no need to change your name legally when your married -- my wife and I were married 11 years ago and she still uses her maiden name. Only problem might be when you get a check to Jane MarriedName when your ID shows Jane MaidenName.

Tort reform limits your ability to seek redress under the law in ALL of the above cases, not just the medical cases. It also limits what you can recover, regardless of what your injuries are. $250k might be hunky dorey for the 85 year old man who was paralyzed when his doc used Lovenox during back surgery, a drug with a black box warning placed by the FDA because it can cause paralysis in back surgery, but $250k isn't so hot for the baby who suffered brain damage at birth because the doc used Cytotec, another drug with a black box warning that warns against its use in L&D. In that case the baby suffered brain damage. The mom died. Bled to death due to a ruptured uterus. Says right on the box not to use it, but doc didn't know that. The brain damaged baby also only gets $250k, but for his lifetime of care, he will need much more than that. By age 3 he will be through that, and then he will be placed on Medicaid, which is paid for by the taxpayer. Of course, if you have no Medicaid, his healthcare will be paid for by the taxpayer. This baby has no ability to ever hold a job, but he still must be cared for. And the mommy who died? Her family didn't get to sue for her wrongful death at all. Dead equals noneconomic damages, and you don't get to sue for that.

Hi there. My name is steve & I got married 3years ago. After the wedding my wife changed her last name and took my middle name because my last name is too long and has a bad memory to me.. I bear one of my fathers name as my last name. He used to abuse me in the most horrific ways that I cant mention.. He,s passed on now.. I hate this name and avoid relating to it.. Infact I hate when peple use my last name when calling me.. On the other side of the coin I love my father In-law and named my son after him.. Do you think I,ll be a freak if I took his name as my last name and dumped my biological fathers name.. And how do I legally do it?. Ps advise.
