Shoot lots of pictures. I used to use a motor drive and shot a brick of photos (24 rolls of film in a brick. I would process my own film so there was no chance of them getting out. Of course with digital those worries are over. I used to shoot off a tripod far away with a telephoto lens so I wasn't in the models face. or private space.. . I don't have personal experience, but I found my friend, who had major depression, seemed addicted to self inflicting pain. It started before I met her, but even now, when she is no longer depressed, her pain reminds her of how she used to be. I would say you can get addicted to it, because when you cut yourself, you are hurting yourself for a reason, and that reason won't go away overnight.

It differs from person to person so I would not comment on particularly this physician who had multiple strokes and has Hypertension because without seeing him, it is impossible to give an opinion about his mental status and competency to make decision. It is his physician and family member who can reach to some conclusion if they feel patient is not competent. If patient is completely fine, in the normal state of mind then definitely his wishes to be DNR must be followed at any cost.
