I'm assuming she isn't illegal, is that correct?. . If she takes her birth certificate, and if she knows her social security number, and has some sort of other id to prove that who she claims to be, she is, go to your social security office. They will issue her a card....

By all means include a brief letter thanking him in advance for his help in completing and sending off the forms, but I don't think enclosing a gift would be appropriate in the circumstances. Sending letters of recommendation and filling in forms about former students is part of his job, really, and if you enclose a gift it might look like you're trying to chivvy him along or even bribe him.. . I see no harm in sending separately a small gift at Christmas if you wish, though, with a card thanking him for his help. I think just a small box of chocolate truffles or mint chocolates would be fine.
