Last year the peer request forms were prewritten and this year I need to write out a request myself. I am drawing a blank. I am looking for some examples of a letter/email asking a coworker for peer feedback. I want something professional and not too specific as there is a specific form that they fill out for different categories.. I am looking for something specifically related to feedback. I've found loads of request letters on line but nothing asking for feedback. Asking someone a favor is not the same as asking someone to tell you what you've done well and what you need to improve on.. If you read the entire question it tells you there IS a form they fill out..

You do not need lawyers. The forms for a name change are very simple. It would, however, help to know what state you live in. Most states require publication in a newspaper. It also sounds like you are under 18 because you say your folks haven't been helpful. If you are underage you will need parental consent from both parents and one of them actually has to file the petition. If you are an adult then you don't need permission. Just go to the civil court and request a name change packet, fill it out neatly and properly, submit the necessary documents, and pay the proper fee. Here in NY it costs $65. You get a court date and they give you a copy of the order to publish in the newspaper. Most newspapers will charges less than $50 for this. After that you bring proof of publication which is a letter from the newspaper stating what you published. And you bring that back to the court and you're all done. All you need to do then is change your ss, drivers license, passport, etc.

Me and my wife got married almost 2 years. In Ohio after we got married we went to the DMV and got her an ID. Her parents are nuts and would not help us with anything. So we did it and showed then our marriage license and social security card. We got an ID of her with her new last name. Sucks because she never had a school picture ID or anything like that. Now a days were trying to get her license. We went to the social security office and tried to get a new social security card but they wont let us get one because she dont have proof her maiden name is really her.. She has an ID of her with her new last name and her birth certificate it shows her maiden name. So they told us either get her transcripts from school or papers from a family Doctor (in which she never had) So we called the school and told them to send it. They did and we never received it. It was over a month ago now. Dont know what happen to it. We now live in Colorado. What other options do we have? There treating us like shes a illegal immigrant..
