middle name change at 3 years old help please?

How would I begin a business email? would I say "dear so and so"?

My ex and I have agreed to him terminating his rights after paternity has been established. I need help writing a notarized agreement before we get started. We are still going to follow through with the court system. I just want to have this for peace of mind and I don't know how to put this together.. This is not to trick him into anything. He has agreed to terminate his rights willingly because he has no interest in stepping up to the plate. Although neglect and abandonment play a big role in the case against him. Also I have my partner would like to adopt my child. I just wanted to have this for peace of mind and proof that he agreed to follow through with the process should anything happen..

Was given a blank form to fill out and don't know who what or why this information is needed. What are my rights.

Well, you can actually start using your new name informally even before the legal name change goes through... so that's one thing. In fact, the legal name change process isn't even strictly required (but I think for all practical purposes it is... like getting a new driver's license, etc.). . One other consideration you should look into is how much it will cost you to change your name in TX vs. your new state... I know California really jacked up the cost of changing your name a few years ago, so the costs can vary quite a bit.. . One other consideration is that it may take longer than you think to get all documents changed over (DMV, SS, passport, bank accounts, credit cards, credit bureaus, etc.). . Another consideration is maybe you can save money by moving, waiting 6 months, and then changing your name and your documents at the same time (you're allowed to keep your old driver's license 6 months after moving to a new state... sometimes longer - so check your state).. . I would call the offices (county clerk?) in both states and get answers to all the above questions, and then it's up to you.
