    You do NOT have to change your name by deed poll to make it legal. You can change it by "common usage". I changed my children's surname by writing a letter/document stating their old name, new name & that I am legally resonsible for them. I checked with a soliciter & it is a legal document. Apart from their birth certificate which cannot be changed I have changed their name on everything. Schools, Child Benefit, NHS records, banks, passport, everything. The document has been accepted because it is legally binding. An adult can do the same.. . I was able to do this with just a letter from myself because I have sole parental responsibility over my children. If your childrens father shares parental responsibility then he will need to be on the letter too or write one of his own. If he does not agree to the name change then it won't be legal.. . I will get the letter out & email you the necasary wording if you like.

Irrevocable means it can't be cancelled- it's forever. Did you ask her why she wants this? It makes no sense & sounds like a scam. There is nothing that could be used as an alternative. She want's to have you give her authorization to do anything she wants in your name. There is no need for her to sign for you- ever. Don't do it.
