    An affidavit is a notarized statement of fact. It really doesn't have to look any certain way to be valid, but they generally go like this: Title (called the header) in all caps, centered across the top. Then the first paragraph gives the deponent's (in this case, your child's) civil information, "My name is (full name), age sixteen, minor child of (your names), who resides at (street address) in the city of ___, ___ county, state of Pennsylvania." Subsequent paragraphs should each be confined to a distinct fact pertaining to reasons for requesting transfer of custody. Then the last paragraph (called the footer) says something like, "(full name), having been duly sworn, deposes that the matters stated herein are true to the best of his information, knowledge and belief." Then leave a line for your child to sign on, and underneath type his full name. Then type "SUBSCRIBED AND SWORN to before me on this _st day of ___, 2008." (You can even type the blanks and fill them in in pen when you sign.) Then leave three lines for the notary's info, and enough space at the bottom of the page for a seal (about a one-inch circle). Take your son to sign it in front of the notary (available at banks and grocery stores for a minimal fee). He will need to bring his picture ID.. . Hint: Let him write his own reasons, just help with grammar, ect. If you write it for him they will hold that against you.. . Good luck.

    My fiance and I have been together for 5 years, he is currently incarcerated. There is a woman claiming him as the father of her 1 1/2 year old son. He was court ordered to take a paternity test.
