I want to change my middle name. What papers do I need to make that happen? I know I need the basic (birth certificate, ss card), but is there any other things I need to change it. I want to get it done on monday..

If there is no father listed, then you can change her name easily in probate court. There is a fee and you will have to go in front of a judge. With the 12 year old, the father will have to be notified for his consent. My daughter doesn't see her father and uses my name, even though his name is on her birth certificate. Unless he agrees, then I cannot legally change it, however, she can use whatever name she wants. When we got divorced, I was advised not to bring it up as I was risking the judge forcing her to use his name when she'd been using my name since she was 1 year old. Good Luck!
