First reference link is to a form that lists candidate attributes that could be addressed in a recommendation letter. The second link is to a sample recommendation letter for grad studies. The third link is to an extensive collection of recommendation letters along with advice on writting them.

I have an older sister called Andrea and she suffers from bi-polar disorder, schizoid disorder and she is very socially secluded. When we were kids, Andy never really had friends and would often study in her room all the time and keep to herself. She has had relationships but for some reason or another they have broken down and she refuses to talk about them.

What websites have examples of home day care contracts?.

Don't bother sending the letter now, unless the laws of your state specifically require it.. . Wait until the name change is complete, and you have a copy of the court order (or other official document) stating that the name change has been approved. Then, send a copy of that to anyone you need to notify.. . If the laws specifically require that you notify your creditors, then just send they a letter saying that you have petitioned to have your name changed. Include your account number or other identifying information, the same as you would for any other correspondence with the company.. . Send it by certified mail, so you have a record that they received the letter, and keep a copy of each one for your own records, so you can show the court.

. Also, just so you know - any company can reject a 2 or 3 weeks notice. Unless you want to collect gov't benefits (which you don't need since you got another job), let her list you as released from the job. When you have an interview in the future, be sure to mention that you gave notice and it was denied.
