We're in Mississippi where the child was born. Husband's changed surname with a court order. Child already was given the new surname at birth before father changed his. Want to amend the child's Mississippi birth certificate with father's new surname. Vital statistics wants a court order. Do we have to file a new order just for a child even though child's surname isn't changing (if so I'm looking online for such a document)? Or can we submit the father's court order with the birth certificate amendment form? Child is not listed by name on father's court order. It says "the petitioner has child(ren)".. I know this sounds odd. At birth the child was given the new last name/family name. At that time, we had had three different last names for each person (father's adopted, mother's maiden, and child's). With court orders, each parent changed the last name to the same last name as the child, so everyone would have the same last name. My husband's/ the child's father's old last name is on our child's birth certificate. We want to update our child's certificate with my husband's/child's father's new last name. The child's name has never changed, nor will it change, just the father's..

His mum was not married to his dad but she took his name and has done for the last 40 years, however, when she went to take her driving test, it is in her maiden name and not her "married name" and the only way she can get it in her "married name" is to change her name legally with the help of a solicitor. This includes all documents eg passport (she doesn't own one) and any other photographic proof as to get one, you need your birth certificate and or marriage certificate to confirm your change of name.
