Where on the internet do I find sample limited model/photographer/agency release form templates?

OK, so really annoyed at Barclaycard, I attempted to reclaim PPI that was 100% mis-sold to me and they have rejected me. I know that I can go to the FOS but wondering if anyone has rang barclay and managed to talk sense in to them.. . I basically was 18 (8 years ago) when I was sold the credit card in a shopping mall where the girl said oh you wont get the credit card anyway and I'll get commission, you will be doing me a favour, I unwittingly allowed her to sign up to what I can only describe as my worse mistake ever, she filled the form in, I actually remember telling her I was on a wage of something like 6k at time, was a student and was only working part time in shop and she just asked me to just sign my name which I did and I remember her filling rest in to the point where I seen her writing 16k in the box, now Barclaycard have came back to me telling me that their "sales advisors were trained" so therefore there was no evidence i was mis-sold PPI. Genuinely never ever ticked any sort of box for PPI. Then stupid 18 year old did max her Platinum card that she did receive in post out and was being charged about 55 a month in charges at time and seen there was PPI on my bill. Obviously my mistake that i maxed out credit card that I gave me, but not only that, I rang up to cancel the PPI of it so when i paid minimum payment i would be paying something off but was told on phone that NO i could not cancel the PPI for whatever reason, think the girl said I couldnt afford to pay it off if i got unwell or something. so persistent i was, I rang a few months later and insisted that it was taken off my account and this time again the girl on the other end of the phoneline wouldn't but changed it so that i wasnt paying a % of the final bill but a set amount which worked out less so I did end up agreeing but in no means did I ever ask or want this on my bill. I did finally get my card paid off and was so happy that I cancelled this card forever!!!. . What a wretched experience and I will never ever use any of their services again, Sales people may be trained but at end of day they target the easy ones for their commision and their sales pitches go out the window.... . Sorry just wondering what you would do if you were me, I have worked out that I probably paid about 700 altogether in PPI, i know its not a lot for most, but it would feel like a small victory for me.... i was 18 i shouldnt have signed but at that point the woman was very convincing, u think barclaycard train their sales advisors to fill in fraudulent credit card forms. On another note i did pay it all offf. but not once was i told about PPI prior to me seeing it on statement and tried my best to cancel when i did.

Word 2007 templates should be saved as .dotx, not .dot. That's why Word 2003 is opening the template and creating a .doc file.. . But don't just change the filename extension from .dot to .dotx. That won't work. Instead, open the .dot template from within Word 2007, then save it as a .dotx.. . Hope that helps.. . PS: Just realized you may have lost some Word 2007 features when you saved as a .dot file. You may have to recreate them
