States differ as to how many witnesses are required, and whether a notary is required. I'd get everything witnessed by three people and notarized, if you plan on using it in multiple different states.

Depends on your nationality and what country you're in and if you have parental consent. As a minor parental consent (the parents on the birth certiciate) is usually required or a court order.. . If you're in the same country you were born in then you can get it done easily though. fI you have legal permanent residence it is possible to get a change of name done in the country you're in (although it depends on the country in question eg no matter what your nationality in Australia if you have Permanent Residence you may change your name with the Registry of Births, Deaths and Marriages and then you can take that evidence of name change to the embassy and they can change your name on your passport).
