Is there anyway to find out someones health history?

I want to change my last name. Not literally - I want to add a last name (not by hyphenating it, but by just having it like say "Garcia Fernandez"), along with changing the spelling of my middle name (not the whole name, just the spelling). Reason? A) Me and my father have the same name and I'm often confused for him especially as far as the legal documents are concerned. Also, Iwas born in Europe, but moved to Canada as a toddler. The spelling of my middle name isn't used in North America (Teodorius - I want to change it to Theodore). I am 18 years old.. . Is this possible for me? How do I do it? Can I sign all the papers by myself or am I not old enough for THIS specifically? Where do I start? How long does this process last & how much does it cost?.

I take it the contract itself does not specifically state when the extension notice is to be provided. If indeed this is true, you have to determine what the intentions of the parties were. When I was admitted to a local hospital recently, the attending physician tried to coax me into having surgery for a benign condition that was unrelated to the symptoms that had brought me to the ER. I explained to him that I already had non-surgical treatment in progress at a different facility and therefore I would not have surgery. Later I found out from my visiting family that I could not go home because I had been scheduled for surgery that evening, even though I had clearly stated I did not want it. I had to get very assertive with the staff, while still hooked up to IV and feeling very debilitated, to get a discharge. The cause of the symptoms that had put me in the hospital was never determined.. . I was shocked, and ever since I've been concerned that, should I be taken to the hospital after an accident, for instance, they would again attempt to do this surgery against my wishes, especially if I was incapacitated. My insurance company suggested that I prepare an Advance Directive to protect myself. I was told this would let me specify treatments I do not want to receive, and even name doctors I don't want involved in my care.

My college is holding a few charity events throughout the year and our first is next month and I am in charge of preparing all of the events. We're doing a raffle and I have to contact local businesses and ask if they would be willing to donate things for the raffle. I'm 99% sure of what I'm putting in the letter but I'm not too sure on how to actually set it out. For example, how many lines to leave between "yours sincerely" (or 'many thanks' etc) and my signature at the bottom. It's just silly little things like that. I have never had to do this before so it is all completely new to me. Any and all tips will be greatly appreciated! :).

My Fiance, my 14 month old son and myself are moving to Vegas around June or July 08. How can I go about setting everything up here in Hawaii? More specific, I mean... finding jobs for the both of us, Medical insurance for all of us, esp. my son. Getting the housing situation secured. First of all, the first two responses sounds more like criticism than advice, our decision to move maybe a bit naive but we certainly aren't stupid. I do know that we need to secure jobs before we make the big move. To answer your question as to what led to our decision to move is, the cost of living in hawaii is too high, and we've heard, correct me if i'm wrong, that Vegas is way more affordable. If you've never lived in Hawaii, then let me give you some figures... a 1998 3 br 2 bath home, about a 35 minute drive from the main city of Honolulu, which by the way is a long drive in Hawaii, is going for $465,000. Needless to say, the closer you get to town the higher the prices go. The second thing is that we have a handful of friends that made the move from Hawaii to Vegas and a few with children. One of which is a 23 year old, who owns his own home and has his wife is a stay @ home mother. Our friends have nothing but great things to say. So are there any more TIPS that can help.
