What legal form would I use for the following?

Legal question not a genealogy question............. . You have separate issues here. . 1. A child who has the right to know who her father is and that he is on her birth certificate. ......Right to know her ancestry, if he signed her birth cert then you need his written signed permission to change her surname. . 2. What you think of her dad. "I thought eventually we would get married but that will never happen because he is a deadbeat." but your bad judgement of his character and decision to have a baby without being married prior is not her fault, nor the fact that he is not the father you thought he might be is reason to change her name...that is more about spiting him.......

What does the contract need to be for?. model release?. Wedding coverage?. a lot more to cover........

My husband's favorites are Mary, Daisy, Thomas, or Joseph. But as I've said, he pretty much likes every name I do.. My mom likes: Eden or Vivian, middle name would be Elizabeth. lol Noah for a boy, she says I need to keep with the Biblical theme. My thoughts are the same on this, though I like more than Biblical boy names. Henry James and Landon Charles are two of my favorites. . Me: I like a ton of different names for a variety of reasons. :-) I'm willing to compromise... But without that I would probably name a child.... Amelia Claire "Millie" or Ethan Robert "Bobby" instead of Mary Elizabeth "Ellie" or Thomas Alan "Tommy". . Now for the boy's we already have... Our first child is David Jackson S. His name means beloved Jack's son and he most certainly is. Jackson is after my husband, they share the same middle name. However, when I found out I was pregnant I wanted either an Elijah James or Emily Jean. lol I came up with those names when I was in 5th grade. Not bad, but I see now they need a bit of work. :-) Elijah James has too much J going on and Emily Jean is cute, but Imogen is a bit more refined. Also I didn't realize until years later that both names bore the same initials. Talk about not paying any attention. hehe Anyway, my second child is Adam Michael S. I really wanted to name him Ethan Gabriel, but my husband had bad associations with BOTH names. I eventually fell for the name Adam and thought it sounded so wonderful with big brother David. My husband loved Michael, so that was that.. . Anyway, I just love names and I am susceptible to outside influences. lol Baby names has been my hobby for many years now.

YOU SHOULD ALSO BEAR IN MIND THAT USUALLY HR PREFER TO SPEAK TO THE PERSON GIVING REFERENCES THAN TO READ IT AS THERE HAVE BEEN NUMEROUS FAKINGS OF SUCH LETTERS SO HUMAN RESOURCES EVERYWHERE PREFER TO TALK TO THE PERSON RATHER THAN READING A LETTER AS THEN THEY CAN HAVE A DIALOG AND FIND OUT THINGS (MINI-INTERVIEW) FOR THEMSELVES.. .
