    Standard form to vertex form:

I've been trying to add a contact/feedback form, to my version of Frontpage 2002. . I know that when creating a website from a template, it's easy to do, because you can just create a Feedback page, but as my website is already an existing one, how can I add one page with the feedback form on?. Or, is it possible to make a form from scratch, that actually works, and customers can send in a request, which will be sent to my email?. . Please help....

In a cover letter it is better to not discuss your lack of experience, but rather to show your interest. I've included a helpful link, but a basic search in Google 'Template/examples of cover letters' will show you the type of thing you should write. You could include your CV with the cover letter. If you get an interview you can explain your lack of experience but give them good reasons to hire you, and examples of what skills you have that can help you learn a new job role despite little experience.

It depends on the circumstance. Normally, "Mr. White:" is used for formal addresses, such as business memos and letters. For example, if you are applying for a job use this form in your cover letter. "Mr. White," is used for informal addresses, such as a letter to a friend or family member. But yes, both are grammatically correct.
