Making medical decisions for your spouse if he or she becomes incapacitated and unable to express wishes for treatment.

Go to your county courthouse and buy the forms for a name change. You'll need to fill them out and file them with the court. Additionally, you'll need to run an ad in the local legal newspaper advising of your intent to change your name. The court will set a hearing date and time. Show up at that time and the judge will ask you some questions to insure that you are not changing your name to try to evade creditors, etc. Most likely, he or she will grant your petition and sign the order. You'll file the order and your name is changed.. . I don't know about other counties, but Los Angeles County has a packet that contains all the forms and instructions. It's not too complex, but if you have any doubt about doing it your self, hire an attorney. It shouldn't take more than a couple hours of his/her time.

My bottom line question is in two parts. 1. Do you have an advanced directive? 2. How do you feel about advanced directives? 3. If you currently don't have one in place How does your family know how to handle this difficult questions? Have you talked to them about it?. . Thank you for your help..

    what is the'power of attorney'?

POA's are not necessarily filed with the Clerk of Courts. Is this a durable power of attorney? The person who is holding it only needs to present it when it is needed to be used ( as in a bank transaction) and the POA is usually copied at that time and kept on file. If this POA is yours that you granted, there is a standard form to remove it as well. As a last resort, you can go through an attorney to remove this,,,,as I had to do in my last divorce. I hope you find this useful.
