I assumed that your name was changed at the courthouse? That is the standard way to do a name change I believe. (i worked in Civil court). In any case, you will have to get in touch with the court, either by going there (if you still live in that town), or by mail. Ask the DMV if they will accept a copy of your "Name Change Decree" certified from the court. When I worked at the court, we charged a $15 fee for this, and at least in California is was certainly sufficient enough for most any kind of government agency. I dont know how your state can differ. What you can do right now though, is get on the website for the state that has your decree's CIVIL Court website. They usually will have "FAQ", and I know this is one that comes up often. Unless you can drive to the court that this was done at, it is going to take a little time for the court to get your request and to process it. All this info, should be on the website. Hit me up and add me to your network, if your court is in Southern Cali. I work for an attorney now, and I am at court all the time, and I would be happy to check on this for you. Let me know.

I'm 12 but I want my parents to change my last name. I have my mom's last name since her and my dad where never married but I want to have my father's last name. My mom is giving me the chance to change it and I would hope to change my last name. What's the process of changing my last name and is it possible to change it even though I'm not 18?.

looking for "pin and cresent joint jig"....?

He should change it now. It takes a while for name changes to go through, as simple as it is. I changed mine when I was 18, and waited for a month to get a court date to change it - and that was in the small state of New Hampshire. If you live somewhere more populated, the court wait will be longer. Then, there's the hassle of changing your social security card, license, bank accounts, which takes weeks to do.. . When you change you last name to his after getting married, they will go off the name on the marriage license and certificate. If it still says Jones, you will have to go through the whole court name change process at well, so basically, you would have to do 2 name changes. If his name is already Smith by the time you apply for a wedding license, it will make it easier for you to change yours to Smith as well.. . Make sure he keeps the official paper he gets from the court when his name change is made official. It should have the court's/city's seal on it. This document will come in handy in the future (trust me, I know), like if you move to a different state and he needs a new license, or you apply for a passport. Whenever he applies for something that requires a birth certificate, he will also need to provide a copy of the name change form, to show why his name now is different than on his birth certificate. Your marriage certificate will cover that purpose for you (But if he waits to change it, you'll have to have your birth certificate, marriage certificate, and then court paperwork).. . So yeah, change it now.

why do you insist on vilifying we who work in the medical field and why do you wish to downgrade us from professional providers to civil servant status, do you not want us to earn a fair wage from our extensive training and hard work? your undermining of my profession has left my family with less resources and has made my job much more difficult. I am considering another field if the Obamacare legislature continues to devaulate my patients and my field..

what is in a business plan and why?
