A child?s name can be changed at any time, provided it is not to deceive or defraud another person. For most purposes, a statutory declaration is generally accepted as evidence of your change of name.. . A statutory declaration is a statement, recording your intention to abandon your old name and adopt a new one.. . Preparing a statutory declaration can be complicated. I'm opening an online clothing store in a couple of months. I've filed for my tax ID. I will be running my business from my house, so it is considered my legal business address and I do not want to put my home address on my website for many reasons. I plan on getting a mail box from a near by UPS and using that address on my website for returns. I asked the man that was helping me fill out the form if this was legal and he didn't know and told me just to put my home address since my business is technically operated from there. He also asked his fellow employee, and he also didn't know.. Is this legal to use the UPS mailbox address on my website for a return address? I don't want to get into any trouble with my business. The UPS mail box address is not listed on my tax ID form..

Honestly, if it's not worth your time to wait on the phone until you get a warm body, then I'm not sure how to help you. I understand it's frustrating and annoying but isn't it worth $14k to stay on the phone for a bit? Get the name of the person you talk to and be prepared for a bit of a battle.. It sounds to me like you need to write a letter to the Child Support Office denying all knowledge of the claim. It's possible that there's a mixup with a Social Security Number or a name duplication. That's where you might start. . If it helps, I work in a law firm and we had a client who received a letter from the IRS saying he owed over $100k in back taxes. He was freaking out, understandably. After I reviewed the letter he received, it was clear that there was a mistake in the SSN. We wrote a letter and straightened it out. . Good luck!
