It totally depends on what you plan to do with the document. If you for instance want to use it as an example in a presentation and you want to change names etc., You don't even need to give credit to anyone. Just make sure it says it is a sample or example and make sure it doesn't get turned in to the government or stated as actual fact.. . Now if it's a case of a Government ID, birth certificate etc., It must have a legible watermark saying SAMPLE across the document, so that it can not be mistaken for real.

Personally, I'd just get my parents to write to the school and make them change the documentation. Your Step father could adopt you but it's FAR TOO much hassle, legal stuff and money involved.
