Marketing job application?

Ok, sorry long post. I bought a doberman puppy three weeks ago. She told me she was the runt.. . When i arrived she was twice as small as her littermates which were sold and leaving that day. They were only six weeks old. The puppies ribs were showing and belly extended. I asked about this and she says probably due to the fact she had wormed her the day before. She said she was eating well and not nursing so she was fine to leave.

I have been spelling my first name two different ways since I could write, and I finally figured out why. The name on my birth certificate has an extra letter than the name on my social security card. I only know now because I just got a job and they required a copy of my social security card. My parents held my birth certificate and security card in fear that I would get an ID and do bad things, I guess. (I turn 21 in a couple of days.) I don't know if this is a huge problem, but if it will mess me up in the long run, I need to know, is it absolutely imperative that I get this fixed? Will it affect anything with my job? I noticed that when they send me letters in the mail for benefits, they use the name on my social security card. . Also, I want to go by the spelling that is on my birth certificate, and not the name with the missing letter on my social security card. That's the name I go by. I figured since my schools never called me on it, and at school our social is your student ID, that it wasn't a problem..

No prescribed form. Write application on letter head / any paper.

I am divorced and have my ex-husband's surname. Now, I am planning to get married second time and I want to change my surname into my new husband's surname. How do I do that?. Do I need to change it into my maiden name prior to wedding? or it's automatically after the wedding that I can take his name? . confused and can't find direct answers online...pls help.
