Does anyone have a generic employment application form they can email me?
Of course, marriage/divorce are the most common ways to change your name. In a divorce, you are given the chance to go back to a maiden name or other previous name from another marriage. A complete name change is not allowed (changing your name from Helena Graham back to the maiden name of Helena Smith is allowed but in this case, Helena Graham cannot change her name to Suzette LaFleur).. . Here in the US, you can change your name to anything you want PROVIDED you are not doing so for an illegal reason or the name is just outlandish. Yes, a judge has to approve but s/he has great latitude.
Can anyone tell me what is ISO 9001 Standards?.
i hope you can provide me a sample recommendation letter for a practicum / on the job training for a bank?
I believe you have to go to Probate Court for name changes but that could vary from place-to-place. Cost depends on where you live also so call and see what you can find out and how to go about it. As far as keeping it private, I'm not sure you can do that but the Courts can advise you best what to do with that.. . How sad that your father is blaming you for his own sins. You would never have HAD to testify if he hadn't done this in the first place. Definitely go to the police department and make a report and let them know what is going on, then ask them if they can send a copy to your father's PO or you notify the PO that the report is on file. This could well get your father's rear back in jail (and it should) because he's breaking the law by making threats.
