How long should be a cover letter?

I decided to advertise with yellow pages. When I met with the sales representative he supplied me with an ad content form including top copy, middle copy and bottom copy. I signed the form under the name aaaaaa _______________ (company name). The sales rep did not supply me with any copies of the form. Only until when the Yellow Pages was printed I realised my advert was at the very last page, which has resulted in no phone calls. I contacted yellow pages and they told me the reason was because the aaaaaa part of my name has been scribbled out on the form. They hold all three copies which they decided to send me photocopies of. To my amazement I saw writing on form and the aaaaaa part of my name had been scrawled out. They are now trying to blame me because after the meeting between me and the sales rep I emailed the design team an idea of how I would like my advert to look. I did not include in the design the small print aaaaaa _______________ (company name) on the bottom left hand corner. The design team do on the other hand charge to modify artwork emailed to them and I did never at any stage confirm my design layout idea was final. Is the email irrelevant? I signed a form under the name aaaaaa ____________ (company name) and they have tampered with it after I signed it. Is this fraud?.

im a 19 year old college student and I want to know where I can apply for free health insurance. I have no parents and I live on my own. I tried to apply for medicaid but I don't qualify because you have to be pregnant or have a child. does anyone know of anything I can do. . Last year the peer request forms were prewritten and this year I need to write out a request myself. I am drawing a blank. I am looking for some examples of a letter/email asking a coworker for peer feedback. I want something professional and not too specific as there is a specific form that they fill out for different categories.. I am looking for something specifically related to feedback. I've found loads of request letters on line but nothing asking for feedback. Asking someone a favor is not the same as asking someone to tell you what you've done well and what you need to improve on.. If you read the entire question it tells you there IS a form they fill out..

Just write it in letter format and tell him why you want or need to change the class and what you plan to do. ex- i quit french correspondence because i left it last minute (i admitted this to him) and didn't have anyone able to help me. I wrote that i might take it next year in class since i'll be back another year.
